Welcome New Patients!

We are here to help you reach all of your oral health goals; we will take the time needed to discuss treatment options and answer your questions at your new patient visit.

Here is what to expect at your new patient visit:

Before your visit, please print the forms below, fill them out, and bring them with you to the appointment.

We are located at 445 Highland Ave in Malden, MA. 
Parking is to the side of the building (to the right if you’re looking at the front of the building)
Please enter the front door, check in with Sam when you arrive, and provide your complete new patient paperwork
We generally are able to see our new patients for both a comprehensive exam and cleaning on the same day. If you are scheduled for both, expect the process to take about 2 hours start to finish.

You will see our hygienist for a gum examination and cleaning
We will also take a full set of x-rays and the dentist will complete an exam.

If you have recent up to date x-rays, please have your previous dentist send them to us at reception@stoneybrook.dental
A comprehensive treatment plan will be customized to you, and we will answer all of your questions and concerns
